Access to Care Partners with UIC’s DEEP™ Program to Support Members with Diabetes

Westchester, IL, September 17, 2024 - The Suburban Primary Health Care Council, operating as Access to Care, is proud to announce its new partnership with the University of Illinois Chicago’s (UIC) Diabetes Empowerment Education Program (DEEP). As a grant partner and certified provider, Access to Care will offer this evidence-based program to its members, nearly 25% of whom have been diagnosed with diabetes.
Developed initially at UIC’s Midwest Latino Health Research, Training, and Policy Center, DEEP is designed to reduce diabetes-related health disparities through a multicultural curriculum. The program includes bilingual, accessible modules on physical activity, meal planning, glucose monitoring, and medication management.
This program is open to anyone interested in learning more about diabetes, including those with pre-diabetes, current diabetes patients, caregivers, and family members.
